Howard is wearing a dickie under his shirt. It’s only the collar and upper center part of a shirt. Back in the day, some people used to wear different shirt versions of a dickie under a sweater or outer shirt. I guess it’s a way to wear a layered look without the layers. Fun Fact: Mayim Bialik, the actress that played TV´s Blossom actually joins the cast later in the show, but don't google her. It will probably lead to spoilers.

The joke about getting the girl who played Blossom on the team is very funny and ends up being meta because she later does appear on this show. At this point the joke was, why don't we get the girls from two old sitcoms, they say that's ridiculous, and then they get the girl from Roseanne. (Leslie being played by Sara Gilbert).
