With the help of allies Lt. Jim Gordon (Gary Oldman) and DA Harvey Dent (Aaron Eckhart), Batman (Christian Bale) has been able to keep a tight lid on crime in Gotham City. Also, I believe the change of Joker's story was also made to reflect his unhinged personality. He's canonically so insane that he doesn't even know his own backstory anymore.

The only source I got on why Nolan decided to change the name mid-way in the trilogy is because he wanted to distinguish it from all the other Batman movies.
