I'd say Itachi certainly did fail in a lot of the things he devoted his life to. His bargain with Madara to join the Akaksuki in exchange for them not attacking Konoha was null and void upon his death, and every plan he put in place to kill Madara and keep Sasuke from being corrupted didn't go as he hoped. What keeps Itachi from "being" a failure is that in the end he acknowledged his mistakes and trusted others to do what he could not.

Also, Neel, the reason Itachi destroyed the crow is Shisui's eye existing is a liability. Think about if Kabuto was able to spam that jutsu. In a nutshell, anything Itachi does is optimal.
