I think it gets somewhat explained later, but basically yeah Killua respects Alluka the most in their family. I haven't heard Alluka being transgender from anyone, but maybe there's a chance of that, not sure. I have another idea, but I'll leave it for later, once more episodes with Alluka are shown.

btw for the he/she thing. a lot of people believe that alluka is actually transgender, and illumi and the rest of the family call her their brother out of disrespect. or you can just say its a translation error if you’re boring
