It took this long for Terasaka to come to realization. Something that other students had earlier. Korosensei talked about his nostril and eyes, when Bitch sensei was introduced. On the other hand Itona and the Other guy especially don't have problems with putting other lives in danger just to succeed. Though Itona is clearly being manipulated - at least to some capacity. His "guardian" isn't a nice person.
