You can ask me on Discord if you're unsure whether your imgur album works or not.
This is what the Google Form will look like, just so you know! You will receive your drawing(s) at the end of January when they're all done! If you don't submit anything, you will not receive a drawing for January. I can't compensate and draw twice as much next month, so please remember to submit your request! (edit to clarify the last part)

Yes! I will include that info in the message, but if you're $46 or $69 tier, you use the same form 2 or 3 times respectively, entering one imgur album each time.
