"The switch in the OST is wild right now" that's the thing, the soundtrack didn't switch up. Tsuioku Mezameru Tamashii, the soundtrack that plays, is pretty sad, it starts out divine but ends up tragic and intense. It's essentially Erza and Jellal's theme because it plays in emotional moments linking to those two, like Rob dying + Erza awakening her magic + Ultear brainwashing Jellal (episode 35), Simon dying (episode 40), Jellal and Erza almost kissing 😭😭 (episode 154) and over here, Rita dying

It's pretty likely Rita died from childbirth, but the true purpose behind all of that was to basically say that Mavis forgot the value of life because of the wars, and she learnt it/remembered it after Makarov was born. I like to think of it as, the curse was weak at the moment and only killed the weakest person there who's body was already biologically done out, which is Rita

Pretty sure it's implied she did kill Rita. But guess we'll never know for sure.

The reason it was Mavis that killed Makarov's mom is because in that moment, she truly understood the meaning of life, and that's what places the curse upon people (Zeref is actually explaining in the monologue of the scene but it doesn't sound that clear.)
