In case you're still confused about his name, Ushiwaka is a nickname. His full name is Ushijima Wakatoshi. In Japan, nicknames are often made by combining half of a person's last name and first name. In this case, they took USHIjima and WAKAtoshi to make USHIWAKA. Same with Mad Dog. His name in Japanese is Kyoutani Kentaro....KYOUKEN which translates to Mad Dog. :) Oikawa is the one that thought of both nicknames.
