It’s a common point of confusion in the anime. The reason some Zoldyck family members call Alluka “he” or “brother” is because they don’t acknowledge her as a person and treat her more like an object or a threat due to the entity inside her, Nanika. Their language reflects their attitude toward her, not her actual gender. However, in the story, Alluka is represented as female, and Killua consistently calls her his sister. The conflicting pronouns are used as a narrative device to show who accepts her and who doesn’t.
